  Key Responsibilities:The RN MDS Coordinator plays a vital role in ensuring accurate resident assessments and the effective optimization of care plans. This position will:
  
  • 
    
  • Lead and coordinate the development and completion of resident assessments (MDS) in line with current regulations and guidelines.
    
  • Ensure the MDS process is completed accurately and within required timeframes.
    
  • Collaborate with the interdisciplinary team to develop personalized care plans for residents.
    
  • Continuously monitor and document residents' progress and responses to care.
    
  • Actively engage in the Quality Assurance and Performance Improvement (QAPI) process to enhance the overall quality of care.
    
  • Maintain clear and effective communication with residents, families, and healthcare providers to ensure seamless care coordination.
    
  • Stay up-to-date on MDS regulations and guidelines to ensure compliance with industry standards.
    
    This role is critical in maintaining high standards of care and regulatory compliance within the facility.

      Qualifications:
      
      • 
        
      • Current RN (Registered Nurse) license required
        
      • RAC-CT (Resident Assessment Coordinator-Certified) certification required
        
      • Strong understanding of MDS 3.0, RAI process, and care planning
        
      • Excellent organizational, communication, and interpersonal skills
        
      • Ability to work independently and collaboratively with a team
